BISHOP HENDRICKEN HIGH SCHOOL

TUITION ASSISTANCE INFORMATION



Parents with children who will be attending Bishop Hendricken for the 2011-2012 school year may now apply for Bishop Hendricken and Diocese of Providence assistance.  All assistance will be distributed based on demonstrated financial need without regard to race, ethnicity, gender or creed.

To apply, parents should visit the following web site:  www.factstuitionaid.com and click on “Applicant Sign In” to submit an application on-line.  Paper applications are currently available at Bishop Hendricken.  To obtain a paper application please have your son visit the Main Office or contact the Tuition Hotline at 739-3450 Ext. 117.  Forms will not be automatically mailed to families that applied for financial aid for the 2010-2011 school year.  The deadline for submitting applications is February 15, 2011.  Filing after this date could result in missed opportunities for assistance.

If you have more than one child who will be attending a Catholic elementary or secondary school next year, you only need to file one application.  Please be sure to list each child and their school on Section 2:  “School & Student Information”.  The cost of the application fee is $25.00 (no matter the number of students listed) payable to FACTS.

In most cases you will need a copy of your 2009 IRS Form 1040 (or 2010 if already completed) and all of your 2010 W-2’s (which are required to be mailed out by your employer by January 31st).  There can be difficulty for a self-employed taxpayer to provide the necessary information to FACTS on a timely basis.  In order to submit by February 15th you should try to schedule an appointment with your tax preparer as soon as possible.  FACTS will not accept faxed applications.

Completing this assessment form will serve as an application for all of the sources available at Bishop Hendricken and the Diocese of Providence.
